| Proceeds support the Ronald McDonald House of Cleveland. Chances to win depend on total number of entries. No more than 15,000 entries will be sold, therefore, each entry has at least one (1) chance in 15,000 to win the Home for Hope. Any monies turned in after the raffle closes will be returned to entrant. |

All mail-in entries must be postmarked by September 18, 2009. All prize drawings will be held at the Street Party at Home sight in the Copper Creek Development on Friday, September 25, 2009. |

Entries are $100 each. All payments should accompany the entry form and be made payable to “The Ronald McDonald House of Cleveland” and mailed to The Ronald McDonald House of Cleveland 10415 Euclid Ave., Cleveland, Ohio 44106. |

The $100 entry fee is not tax deductible for Federal or State income tax purposes as a charitable donation. |

Winners’ names will be made available to the media and listed on www.clevelandrmh.org and www.rjperritthomes.com. Winners consent to use of their names and photographs without compensation for publicity purposes to help promote RMH of Cleveland programs. |

No substitute for prizes, however RMH reserves the right to replace prizes listed in this brochure with prizes of equal or greater value should they become available prior to September 18, 2009. |

Winners are subject to all Federal and State income taxes based on the market value of the prizes awarded. A 25% tax withholding will be collected and forwarded to the IRS in advance of fund distribution to winners over $5,000 in accordance with tax laws. All winners must supply their social security number and fill out a W-9 form to claim their prize. |

Winner is responsible for delivery outside the state of Ohio, should the prize be applicable. |

Employees and Trustees of the Ronald McDonald House of Cleveland, employees of R.J. Perritt Homes, employees of McGladrey, and immediate family members living in the same household (or can be claimed as a dependent for federal income tax purposes) of individuals in the preceding categories are not eligible to win. |
